What You Can Do with MyChart

MyChart SGMC Health allows you, and those you allow, access to parts of your SGMC Health medical record. Patients can use MyChart to:

  • Access your health information securely on your computer or mobile device
  • Send secure messages and attachments to your SGMC Health outpatient care team
  • Read your after-visit summaries
  • Read your provider notes
  • View your test results (labs, radiology, etc.)
  • View your images in MyChart through Pocket Health
  • View your released medical records
  • Assign a preferred pharmacy
  • Add and/or Update your Advanced Directives
  • Request new prescriptions online
  • View billing statements
  • Pay bills online
  • Request financial assistance when eligible
  • Complete questionnaires
  • Confirm appointments in advance
  • Check-in online for your appointments
  • Update your personal information, medications, known allergies, and more
  • Online scheduling with primary care providers

Two-factor authentication makes MyChart even more secure. Two-factor authentication is optional. When you log in, confirm your identity with a verification code send to your email.

Please remember when you review your test results, by default, some results might be released before your care team has had a chance to review them. If you let your care team decide, certain results will be delayed so the care team has time to review before they are released to SGMC Health MyChart. In MyChart, you can select if you would like to receive your test results 1) as soon as they are available or 2) let my care team decide.

Sign Up for MyChart

Any adult patient who has been served at SGMC Health can sign up for MyChart SGMC Health.

  • When you visit SGMC Health, you’ll receive an activation code on your after-visit summary.
  • If you do not have an activation code, you can sign up on the SGMC Health login screen. Please note that you will be asked to provide some personal information to create the account.
  • Learn about parental access to children’s medical records at SGMC Health.

MyChart Access for Parents of Underage Children

SGMC Health strongly encourages and supports parental involvement in the health care of their minor children and strives to efficiently facilitate the parental right to access information about their child’s health care. Parental refers to a parent or legal guardian of the child or children.

Children Under 15 Years Old

Parents of younger children under the age of 15 years old can sign up as a MyChart proxy. When a parent signs up as a MyChart proxy, they will gain full access to their child’s account on the SGMC Health MyChart patient portal. The SGMC Health MyChart patient portal is a secure online resource to access medical information. Parents or legal guardians can request access online within your personal SGMC Health MyChart. Please follow the steps below to request poxy access.

  • After logging in to SGMC Health MyChart, click the “Menu,” then select “Request Access to a Minor’s Record.”
  • Complete and submit the form to request proxy access.
  • Once proxy access is granted, you will receive a message in SGMC Health MyChart, which can take up to five business days.

Teens

Parents of adolescent patients, ages 15-18 years old, may also request SGMC Health MyChart proxy access through the same process stated above.

We encourage patients to be active participants in their health care – and this includes engaging young adolescents to access, understand, and use the patient portal. We appreciate their supportive parent’s/legal guardian’s involvement. The SGMC Health proxy role for teens will be limited. Parents will have access to the following features: the teen’s scheduling tools to view their upcoming appointments and messaging with billing and insurance.

The most efficient way to access your child’s medical record is through our medical records department. Because state and federal confidentiality laws prohibit health care providers from sharing certain information with parents without consent, only limited information is available within SGMC Health MyChart. You can complete a medical record request to receive your child’s medical record.

SGMC Health encourages MyChart self-sign up starting at age 18.

MyChart Mobile App

MyChart is available for your mobile device. In your mobile device app store, select the MyChart mobile app from the developer called Epic Systems. Download the app, select “SGMC Health” and log in with your MyChart SGMC Health account information.
