The kit, with easy to use templates and educational materials, is available to community cancer centers online at www.pearlmansurvivorshipkit.com.

While the current focus is on survivorship care planning, that is only one component of a much broader palette of essential post-treatment services. The Pearlman Cancer Center at SGMC is proud to be an innovator bringing survivorship services to community cancer centers nationwide. Please contact us for more information.
