Community Health Needs Assessment
SGMC and local community health partners are dedicated to working together towards the vision of creating a healthier community.
In FY2016, South Georgia Medical Center commissioned Legacy Consulting Group to conduct a community-wide Community Health Needs Assessment (CHNA). The study was conducted to identify the most important health interests of residents of Valdosta, Lowndes County, and surrounding counties.
The CHNA looks at health status, barriers to care, and other demographic and social issues affecting people and organizations throughout Lowndes County and surrounding counties. The assessment helped SGMC make informed decisions about the future of healthcare in Lowndes County.
A task force of hospital employees and community members prioritized the health risk indicators and developed an Implementation Strategy to be carried out by SGMC and partner organizations.
