This event is open to workforce in licensed or exempt childcare programs, Pre-K, and K-12 education in Georgia. To receive your COVID-19 vaccination, you must show valid identification and complete the following steps.

Print and complete the Vaccine Consent Form and bring it to your appointment.
Print and complete the Conditions of Admission Form and bring it to your appointment.
